You may use this form to request video(s) that you need for instruction. This form should be used for those videos that are listed in the faculty guide as recommended videos. Please request your video within 5 to 30 days prior to the class you will be using it. A postage paid envelope will be shipped with the video that will allow you to return them expeditiously to OCLS.
